The  NuLiving Wellness and Longevity Center's  Interventional Endocrinology and Wellness Program will provide to you essential and vital information about your health status and will allow you to follow a " long term health blue print "  individually developed for you from our evaluation.  The testing you will receive is cutting edge and generally unavailable in the typical doctor's office.

For most adults,  we recommend the  Comprehensive NuLiving Wellness and Longevity Center's  Physical and Wellness  Evaluation. This includes determination of your biological age as determined by an H-Scan , full cardiovascular  risk evaluation including the LDL particle size and atherogenicity  index,  metabolic markers for inflammation, methylation , glycation, oxidative stress ,free radical activity, and omega 3 ,  vitamin and antioxidant protection levels.

Standard blood chemistries, complete blood count and urinalysis are performed along with body fat measurement.  Comprehensive hormonal evaluation ,age and sex appropriate,  will be  obtained.  Important  measurement of neurotransmitters, minerals and heavy metal toxins, liver detoxification capacity further enhances your wellness evaluation.

Finally, NLOWLC,PLLC has been able to arrange a significant discount for blood work for patients who have no insurance or a high deductable . In many cases , patients are better off paying cash for a discounted rate for lab than going through their insurance policy. If you are interested in this arrangement, please call for information.

NLOWLC,PLLC charges for all contacts with Dr. Terlinsky pertaining to on going program management. If patients agree to the following schedule of program followup, brief phone calls to Dr. Terlinsky and staff are not charged. In addition , Dr. Terlinsky offers an alternative arrangement of a monthly fee for 100 dollars per month. Patients electing this arrangement will be given a special secure e-mail to communicate with Dr. Terlinsky and his private cell phone number for 24/7 access to Dr.Terlinsky.

First Year: Suggested followup visits in person or by phone (for out of town patients) at 1,2, 3  , 6 and 12 months.

Initial Comprehensive Medical History and Physical.

Insurance accepted minus deductibles and co-payments for the Medical and History and Physical provided by Alan S. Terlinsky MD PC. and . EKG , Pulmonary Function test, Chest X-ray, CV Profiler exam of arterial flexibility, audiogram, and optional 24 hour ambulatory blood pressure monitor. Standard blood and urine lab tests ordered will go through medical insurance and the appropriate contracted insurance company preferred laboratory. (usually Lab Corp or Quest) . Diagnosis and symptoms that are present will be used to help gain entitled, legal medical benefits. Checks and Visa credit cards are accepted.

NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) and Anti-aging/Restorative Medicine Program (AARMP) program charges are NOT submitted to insurance as NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) has no contractual arrangements with any insurance company. NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) does not file insurance . NLOWLC,PLLC will provide a general statement to seek reimbursement from insurance company , flex accounts or HSAs . There are no adequate codes which describe the services provided by NLOWLC,PLLC. Many of the services are preventative often NOT reimbursed by typical insurance companies. NLOWLC,PLLC is a form of complementary and alternative medicine. Many insurance companies do not recognize the services provided by NLOWLC,PLLC or consider these services experimental and not meriting insurance company reimbursement. NLOWLC,PLLC will not provide any records or communications to insurance companies. All records provided by NLOWLC,PLLC will be associated with an administrative fee. Patients are advised to read about the policies of NLOWLC,PLLC and patients will be asked to acknowledge in writing the NLOWLC,PLLC at the time of joining NLOWLC,PLLC program.

Global Assessment Fee & Deposit: For those patients who do not have health insurance a set  fee of $350.00, is offered for the  medical history & physical and diagnostic services.
A non-refundable deposit of $200.00 is required when an appointment is made. The reason for this fee is to reserve the several hours of the doctor's time required for your foundation session. This deposit can be applied to your first visit.
