Alan S. Terlinsky MD PC is a distinct medical corporation which DOES participate with many insurance companies, thus services for the basic history and physical examination and certain diagnostic tests provided by Alan S. Terlinsky MD PC are submitted to those insurance companies with which Alan S. Terlinsky MD PC has contracts with including Medicare.

ESP Club member patients will be able to use their health insurance for some services provided by Alan S. Terlinsky MD PC. Those services are basic office visits, annual history and physical exams and typical medical diagnostic tests such as EKG, Chest x-ray etc. All other enumerated services and amenities comprising the ESP Membership Club are not subject to insurance company or Medicare reimbursement and are included in the yearly fee paid to NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) for the Nuliving ESP Membership Club and its services and amenities. NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) is a distinct corporation and is a separate entity from Alan S. Terlinsky MD PC. NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) has no contractual arrangements with any insurance companies or Medicare. None of the services provided by NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) are provided under contract to any insurance company or Medicare. Therefore , NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) has no obligation to file claims with any insurance company or Medicare and does not provide any documents or codes which are often asked for by insurance companies or Medicare.

ESP Membership Club is an individualized, customized health and wellness optimizing program that stresses early detection and preventive medicine providing concierge service . The ESP Club uses new technologies and tests along with extensive screening tools to create a Personalized Wellness Plan which are features not provided by HMOs, PPOs ,Medicare and other insurance company plans.

The Nuliving MD Extra Special Patient (ESP) Membership Club (NLMDESP MC) is a program comprised of certain services and amenities which is provided by NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) . The ESP Club Program fees are NOT submitted to insurance as NuLiving Optimal Wellness and Longevity Center,PLLC (NLOWLC,PLLC) has no contractual arrangements with any insurance company. The NLOWLC,PLLC and the ESP Club will provide a general statement of services provided. Some member patients may be eligible for flex accounts or HSAs reimbursements but the ESP Club makes no guarantees that any of its fees for its services and amenities will be eligible. . ESP Club members should check with their plan administrators to determine eligibility.

Nuliving MD Extra Special Patient (ESP) Membership Club (NLMDESP MC) is not a health insurance entity such as Blue Cross, a PPO or an HMO.

Nuliving MD Extra Special Patient (ESP) Membership Club (NLMDESP MC) has no insurance company contractual agreements, nor does it have any contractual agreements with any government entity such as Medicare or Medicaid

Regular Annual Fee after Jan 31, 2010 : $1800

ESP Club New Member Time-limited Bonus* $1550 (Before Jan 31, 2010 )

*The annual fee for the Nu-Living ESP Membership Club is $1800. New members signing up before Jan 31, 2010 will receive a "" year discounted annual rate of $250 and a $50 supplement credit each year for the first 2 years. Thus the membership fee for 2 years will be guaranteed at $1550 plus $50 supplement credit each year for the first 2 years.
