Initial Comprehensive Medical Consultation History and Physical. Insurance accepted minus deductibles and co-payments for the Medical Consultation and History and Physical. EKG Lab Tests ordered will go through medical insurance and appropriate contracted insurance lab. Diagnosis and symptoms that are present will be used to help gain entitled, legal medical benefits.

Comment: This is the longest visit of the program. It is an intensive evaluation providing one on one care and attention for the patient which lasts for several hours. Dr. Terlinsky and his staff devote this session to a detailed analysis of a patient's past medical and weight history by integrating the results of detailed and powerful screening material with a focused and thorough interview with the weight control patient. Lasting several hours, Dr. Terlinsky and staff are able to provide the time to his patient which is extremely important to success in weight control but often missing in today's impersonal and assembly-line medical office environment created by "Managed Care".
During this intensive session the long term weight control plan is designed by Dr. Terlinsky with patient participation. After a nutrition plan and strategey are introduced, other important components of weight control are discussed. These include the possible use of appetite modifying medications and nutrient supplement recommendations. The patient has his or her present metabolism defined and explained to aid in the education of the patient about present risk factors, parameters which will be followed to assure healthy body fat loss, appropriate long term weight goals and realistic rate of body fat loss - the beginning of the body weight and fat control process. Lifestyle and dietary change readiness are also explored.
Preparing for Long-Term Success:
- Patients are prepared for success in the weight control process.
- Educational materials are provided as well as suggested reading material specific to the patient's unique problems.
- The patient is also given initial parameters for activity and exercise requirements needed for weight control and tools for self-monitoring.
- Meal replacements and supplements with detailed instructions are explained and introduced into the patient’s dietary program.
- Menus, recipes, food lists and other resources are provided to assist the patient in planning varied and tasty structured meals.

Note: Essential to Dr. Terlinsky’s Weight Control Plan is the use of Meal Replacements, and Nutritional Supplements. These delicious functional foods provide many benefits including calorie and portion control, reduction of the need for tedious food preparation, an easy method of providing low fat, low calorie protein sources; an excellent way to create balanced, daily calorie intake, blood sugar and appetite control, and many others. Research has shown that meal replacements result in better weight loss rates and more consistant weight loss. These products come in the form of shakes, pudding, bars, soups and other styles, to be used as complete meal replacements or as supplements to meals or as planned snacks. We carry several different types and forms such as powder packets, powder shake bottles, and pre-mixed, ready-to-drink products.
While some of Dr. Terlinsky’s programs are without the use of these products, it is expected and recommended that most patients will use at least three of these products per day. Those patients who are not willing to use these products may not be appropriate for this program. Other meal plans consist of 4 to 7 products per day. A very low calorie diet (VLCD) uses only meal replacements. This program requires much more medical supervision.
The basic fee for On-Going Weight Loss is based on three products of the Nutrimed family. The use of other products such as the meals to go or premixed shakes are priced higher. If more than three products are used, the monthly charge will also be more.
NU-LIVING WEIGHT MANAGEMENT PROGRAM STRUCTURE AND FEES
Nu-Living Weight Management is a total program of services and products. The overall program length is variable, and organized into twelve-week blocks which are divided into monthly units for the components listed above.
MONTH 1: Includes the lengthy Program Initiation Visit (see above) Fee ($575.00) in addition to the ongoing Monthly Weight Loss Component Fee, ($280.00)for a total first-month cost of $855.00 (plus supplement fees).
MONTHS 2 & 3 - consist of the Monthly Weight Loss Component Fees ($280.00 each), bringing the 3-month total program cost to $1,415.00. Please note that this cost of $1,415.00 does not include additional meal replacements, vitamins or nutraceutical costs. Payment is expected at the beginning of each program month.
Note: the term “MONTH” refers to any 30-day period starting at the first appointment date - so the “PROGRAM MONTH” is not necessarily the same as the “CALENDAR month”. A patient starting his or her program on January 15th, would have PROGRAM months beginning the 15th of each month.
For patients who are willing to pre-pay the entire 3-month program cost at the time of the first visit, a 10% discount will be applied - bringing the 3 month program cost to ($756.00 ), pre-paid. After 3 months, patients in the active weight loss phase may elect to continue month-to-month at the usual monthly fee of ($280.00 ); OR, pre-pay another 3 month block at a fee of ($756.00 ).
All monthly fees or 3-month pre-paid fees are NON-REFUNDABLE. While we will do everything possible to reasonably accommodate the scheduling of appointments, NU-LIVING WEIGHT MANAGEMENT requires a 1-Week Cancellation Policy for monthly visits and for the initial visit. All INITIAL APPOINTMENTS REQUIRE A $200.00, NON-REFUNDABLE DEPOSIT TO CONFIRM AND RESERVE THE APPOINTMENT.
Accepted: All Major Credit Cards are accepted (Not American Express) and Cash. Insurance is not accepted.
Program Text Books And Manuals: $25.00-30.00.
Vitamins And Other Supplements: Variable Cost...Not Included In Basic Fees.
Will Health Insurance Pay for NuLiving? The NuLiving Weight Management program does not participate with health insurance plans. This means that the following is not submitted to insurance:

Medical services provided by Alan Terlinsky MD PC are submitted to insurance. Examples include History, Physical examination, and some tests performed during your physical. Checks and Visa credit cards are accepted.
Global Assessment Fee & Deposit : For those patients who do not have health insurance a global fee of $350.00, is offered for the initial assessment including medical history & physical and the above described bariatric services. This does not include the monthly fees or fees for the monthly supplements. This global fee is provided by Nu-Living Weight Management.
A non-refundable deposit of $200.00 The reason for this fee is to reserve the several hours of the doctor’s time required for your foundation session. This deposit can be applied to your first visit.
